First-pass clean claim rate is one of the clearest indicators of RCM health. Every rejected claim adds days to A/R and consumes staff time that should be spent on higher-value work.
Before submission, confirm active coverage, benefits, and patient responsibility estimates. Eligibility failures remain one of the most common — and most avoidable — denial categories.
Next, validate coding: diagnosis specificity, CPT/HCPCS accuracy, modifiers, place of service, and provider NPI mapping. A short pre-bill scrub beats a long denial appeal.
Submission hygiene matters too. Duplicate claims, missing attachments, and incorrect payer IDs create noise that slows payment posting and patient statements.
Use this checklist weekly as a coaching tool with your front desk and clinical teams. Small process upgrades compound into faster reimbursements across the month.
